## Description

We are seeking a Physical Therapist for our Outpatient Rehab department in Lake Villa, IL. As a Physical Therapist, you will interpret physician referrals and complete evaluations using appropriate methods. You will establish a plan of care and implement individual treatment based on evaluation results and functional goals. Responsibilities include monitoring and evaluating outcomes, adjusting treatment plans, and communicating with physicians and other teammates.

The ideal candidate will have 1-2 years of experience in Pelvic Health, Vestibular, or Lymphedema. You will need to have excellent communication, interpersonal, and organization skills, as well as intermediate computer skills.

Responsibilities:

- Interpret physician referrals and complete evaluations

- Establish a plan of care and implement individual treatment

- Monitor and evaluate outcomes and adjust treatment plans

- Complete required documentation and maintain productivity standards

- Participate in performance improvement activities and pursue professional growth

Requirements:

- Doctorate degree in Physical Therapy from an accredited school

- Licensed Physical Therapist in the state of Illinois

- Basic Life Support (BLS) certification

- Intermediate computer skills and excellent communication, interpersonal, and organization skills
