## Description

We are seeking a full-time Physical Therapist to join our team at Advocate Health in Charlotte, NC.

The Physical Therapist will evaluate patients, select and administer assessment techniques, and synthesize information to establish an appropriate patient care plan.

Responsibilities:

- Evaluate patients and develop care plans

- Gather and interpret data from medical records, physician reports, and patients/families

- Establish functional goals and provide evidence-based interventions

- Document evaluations, progress notes, and discharge notes

- Communicate with physicians, referral sources, and clinical staff

- Supervise PTAs and Rehab Tech/Aides as required

Physical Requirements:

- Hearing and vision adequate for communication and reading

- Intelligible speech and adequate language/cognitive skills

- Ability to sit, stand, walk, climb stairs, kneel, twist, bend, crouch, crawl, and reach overhead

- Lifting of patients, equipment, or supplies up to 20 pounds frequently and 50 pounds occasionally

Education, Experience, and Certifications:

- Bachelor's degree in Physical Therapy

- Applicable state license for Physical Therapy

- BLS certification
