## Description

An exciting opportunity has arisen for a Band 7 Specialist Tissue Viability Clinical Lead within Barking & Dagenham and Havering Tissue Viability Team.

The post holder will lead the Tissue Viability Team and be responsible for the management of the service caseload ensuring the provision of a high standard of nursing care to patients and their families in the community.

Key responsibilities include:

- Holistic assessment, planning, implementation and evaluation of care

- Nurse Prescribing/Non-Medical Prescribing

- First contact assessment, care and referral

- Providing public health, health promotional programmes to improve health and reduce inequalities

- Maintaining contemporaneous written and computerised records

- Providing support to the Tissue Viability Team and wider CHSCS colleagues

The salary for this role is £49,387 to £56,515 a year.
