## Description

Join our team as a Home Health Physical Therapist and play a vital role in helping clients/patients/residents recover and improve their mobility, relieve pain, increase strength and prevent deformities.

As a Home Health Physical Therapist, you will conduct examinations, evaluations, and interventions for clients/patients/residents affected by injury or disease. You will utilise tools and techniques to administer physical therapy interventions following safeguards, document client's/patient's/resident's information for evaluation, establish therapeutic plans, and modify them if necessary.

You will educate clients/patients/residents and their family members about treatment plans/schedules and self-exercises to be continued at home. You will also demonstrate knowledge of and utilise appropriate age-related treatments and evaluations relating to the needs of clients/patients/residents.

This role requires a strong understanding of current research and its application in daily treatment of clients/patients/residents. You will work with individuals of all ages with varying diagnoses and disorders, demonstrating flexibility with acceptance of variable work schedules/assignments.

As a Home Health Physical Therapist, you will participate in activities to promote the department, Good Samaritan Society, and Sanford Health. You will interact with individuals outside of the department, including providers, case managers, third-party payers, public organisations, companies/contractual partners, etc.

You may be asked to supervise assistants, aides, and students as they provide client/patient/resident care, treatment, or other services. You will be expected to provide ongoing education and training to peers in the area(s) of expertise as determined by the department leadership.

Based on facility needs and assigned locations, you may focus on those receiving intervention in the post-acute, long-term care, and home health care environments.

**Qualifications**

- Appropriate education level required in accordance with state licensure.

- Must be licensed in the state(s) of practice as a Physical Therapist.

- When applicable, may require valid driver's license.

- Obtains and subsequently maintains required department-specific competencies and certifications.

- Basic Life Support (BLS) for healthcare providers required or within the first 60 days of employment.
