## Description

We are seeking a License Physical Therapist Assistant (LPTA) to join our Home Health team at Advocate Health in Charlotte, NC. As an LPTA, you will play a crucial role in managing patient care, implementing therapy programs, and collaborating with our multidisciplinary team.

**Job Summary:** The successful candidate will be responsible for managing assigned patient caseloads, collecting health data, implementing care programs, and documenting patient progress. You will work closely with physicians, physical therapists, and other healthcare professionals to ensure high-quality patient care.

**Responsibilities:**

- Manage patient caseloads, ensuring timely care and documentation per plan of care and MD orders

- Collect health data through home assessments, telephonic contacts, and EMR

- Implement care programs, including modalities, procedures, exercises, and education per PT plan

- Document and execute plan of care, addressing patient/family needs, discharge planning, and resource use

- Apply clinical reasoning and skills for accurate assessments, therapy effectiveness, and prioritization of needs

- Provide patient/family education on deficits, goals, techniques, and home program with equipment use

- Observe, record, and report treatment reactions or condition changes to physician/PT

- Communicate updates on schedules, patient data, and orders with multidisciplinary team

- Incorporate short- and long-term goals into therapy, documented in visit notes

- Support collaboration with physician and PT leadership for timely feedback on patient condition or care changes

**Requirements:**

- Graduate of an approved Physical Therapy Assistant (PTA) program

- Current state licensure as a PTA

- Maintain current state driver's license and auto insurance

- Personal vehicle must be well-maintained and in safe driving condition

- 1-year experience as a licensed PT in a clinical care setting or home health (preferred)

- Basic Life Support for Healthcare Provider (BLS HCP) from AHA (required)

**Physical Requirements:**

- Ability to walk, stand, sit, lift, reach, stoop, bend, push, and pull

- Lift and support 50 pounds in handling patients, medical equipment, and supplies

- Travel extensively to assigned patients' residences

- Work in various home environments and conditions
