## Description

Join our innovative float team at Aurora as a Float Therapist. You will be provided with training and resources to treat patients at outpatient clinic locations within 45 minutes of your home.

**Responsibilities:**

- Interpret physician referrals and complete evaluations using appropriate methods.

- Establish a plan of care and implement individual treatment based on evaluation results and functional goals.

- Monitor and evaluate outcomes, adjusting the treatment plan as needed.

- Plan for and complete patient discharge, including providing written home programs and patient/family teaching.

- Complete all required documentation, including patient progress notes and discharge summaries.

- Clean up after patient treatment and maintain a neat and orderly workspace.

- Demonstrate skill in use of modalities, exercise equipment, and other devices.

- Participate in performance improvement activities and manage your assigned caseload.

- Pursue professional growth through continuing education courses and department inservices.

**Requirements:**

- Physical Therapist license issued by the state in which you practice (Wisconsin or Illinois).

- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).

- Bachelor's Degree in Physical Therapy.

- Intermediate computer skills, excellent communication and interpersonal skills, and demonstrated organization, decision-making, and problem-solving skills.
