## Description

We are seeking an experienced Practice Nurse with an interest in chronic disease management to join our team on a part-time basis.

The successful candidate must be a NMC Registered Nurse with experience of working within Primary Care.

Main duties of the job:

- Child Health/Child Protection, including immunisations

- ECG recording

- Mental Health reviews

- Undertake Chronic disease management and prevention: Asthma, COPD, Diabetes, Hypertension, IHD, Stroke, LD

- Undertake opportunistic screening of BP checks, height, weight, BMI, urinalysis, glucose monitoring and smoking status

- Administer injections under an individual prescription or patient group direction

- Cervical cytology

- Perform wound care using aseptic technique

- Removal of sutures and clips

- To undertake home visits

- Venepuncture

- Administration

- Women's Health

- Assist in the implementation of the Quality and Outcomes Framework (QOF) and Enhanced Services contracts

- Travel Vaccinations

- Stock management

- Infection Control

The role will ideally cover the following clinical areas:

- Cervical cytology

- Venepuncture

- Childhood immunisations

- Electrocardiograms

- Conduct annual reviews and monitoring for patients with chronic diseases

- Administer vaccinations and immunisations in accordance with national guidance and Patient Group Directions (PGDs)

- General Injections

- Basic Travel advice and vaccinations

- Wound care and suture removal

- Diabetes management

- ECGs and blood pressure checks

- NHS health checks and New patient health checks

- COPD management

- Asthma management

- Contraception

- Assist with implants and coil procedures

- Wound care

- QOF and Enhanced Services

Knowledge of EMIS would be an advantage.
