## Description

We are seeking an Advanced Social Work Practitioner to join our Hospital Team. As an experienced Social Worker, you will lead practice, support colleagues, and work with complex cases within Adult Social Care.

The role involves managing a caseload of highly complex cases, providing professional leadership, and expert advice across the service. You will promote strengths-based practice and person-centred approaches to assessment and intervention, supporting safe and timely hospital discharges.

Key responsibilities include:

- Managing complex adult social care cases, including safeguarding, mental capacity, and risk management issues.

- Providing expert advice and consultation to social workers, managers, and partner agencies.

- Promoting strengths-based practice and person-centred approaches.

- Supporting safe and timely hospital discharges.

- Providing coaching, mentoring, and professional supervision to social work staff.

- Undertaking direct observations of practice and contributing to workforce development.

- Supporting practice improvement and embedding learning from research and legislation.

- Chairing professional meetings and contributing to service and partnership initiatives.

- Preparing complex reports and contributing to legal and statutory processes.

- Building strong relationships with health partners and other agencies.

Requirements:

- Recognised Social Work qualification and registration with Social Work England.

- Significant post-qualifying experience within Adult Social Care.

- Experience working with adults with complex needs.

- Excellent knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding legislation.

- Experience managing complex casework and making sound professional decisions.

- Strong communication, assessment, and report-writing skills.

- Experience supporting the development of other practitioners.

Desirable qualifications include a postgraduate qualification in a relevant social work discipline and Practice Educator qualification.
