## Description

We are seeking a Physical Therapist (PT) to join our Home Health team in the Far South Suburbs. As a Physical Therapist with Advocate at Home, you will have the opportunity to work independently, assessing and addressing patients' mobility, safety, communication, and equipment needs.

**Responsibilities:**

- Assess, plan, implement, and evaluate physical therapy treatment programs for home care patients.

- Travel extensively within designated geographical boundaries.

- Work rotating weekends and holidays.

- Provide treatment on standard beds in home environments.

- Lift patients using standard pivot transfers or Hoyer lifts.

**Requirements:**

- Licensed Physical Therapist in Illinois (or eligible for licensure).

- 1 year of Physical Therapy clinical experience.

- Proficient use of home-based therapy equipment.

- Basic computer skills (Microsoft Word, email).

- Strong documentation and interpersonal skills.
