## Description

We are seeking a Care Coordinator to join our team. As a Care Coordinator, you will report to the Manager of Care Coaching and work with Wisconsin Family Care Program members in your community. Your responsibilities will include:

- Assessing members' health and safety needs to develop a comprehensive Member Care Plan (MCP)

- Conducting regular health assessments

- Coordinating with acute and primary care providers

- Educating members on disease processes, nutrition, exercise, and lifestyle modifications

- Documenting in member files within required timeframes

- Participating in member monthly phone contacts and traveling to attend in-person visits

- Working remotely from home and traveling approximately 40% of the time to visit members

**Requirements:**

- Associate degree in nursing

- Licensed Registered Nurse in the state of Wisconsin (in good standing)

**Preferred Qualifications:**

- 1 year of Care Management experience

- Residing within 45 minutes or less of the assigned coverage area

**Additional Information:**

- Location: Milwaukee, WI

- Travel: Up to 40%

- Typical Workdays/Hours: Monday through Friday, 8:00 am – 4:30 pm CST

- Pay Range: $71,100 - $97,800 per year

- This job is eligible for a bonus incentive plan
