## Description

We are seeking a qualified Social Worker to join our Discharge and Assessment Team in Newham. As a Social Worker, you will be working with adults to arrange discharge from hospital in conjunction with the discharge hub and carry out Care Act assessments, support plan or reviews in the community to ensure appropriate ongoing support is in place.

The team works closely with key internal and external partners, including health, housing, and voluntary organisations to promote and support the independence and wellbeing of those in need of support.
