## Description

We are seeking a Clinical Pharmacist to join our team at Sarum Road Hospital in Winchester. This is a full-time role for 37.5 hours a week, with a flexible shift pattern.

As a Clinical Pharmacist, you will conduct medication reviews for inpatients and outpatients, identify interactions, contraindications, and opportunities for medicines optimisation. You will provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.

Key responsibilities include:

- Conduct medication reviews and identify areas for improvement

- Provide patient-specific advice on treatment plans

- Support safe prescribing through reconciliation and clinical screening

- Review and authorise prescriptions in line with clinical guidelines

- Promote safe and cost-effective use of medicines

- Collaborate with procurement and pharmacy technicians to ensure medicine availability

- Monitor and report medication incidents and near misses

To be considered for this role, you should have:

- A degree in pharmacy (MPharm, BSc, or BPharm)

- GPhC registration as a Pharmacist

- Previous experience in a clinical hospital pharmacy role

- Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation

- Excellent communication and clinical decision-making skills

- Ability to work autonomously and as part of a multidisciplinary team

- Confidence using electronic prescribing and medication administration systems (e.g., EPMA)
