## Description

We are seeking a qualified and experienced Social Worker to join our Adult Social Care team in Croydon. As a Social Worker, you will be working with vulnerable adults, their families, and representatives, providing support and guidance to help them lead healthier and independent lives.

The successful candidate will be a qualified and registered Social Worker with over two years of post-qualifying experience, ideally having completed their Assessed Year in Supported Employment (ASYE) as a newly qualified Social Worker.

The role is primarily case work focused, but includes community liaison through GP Huddles and ICN+ Partnership, working with Health & the Voluntary sector. You will be required to develop links and networks within a locality setting, utilizing the various support options available from us and our partners.
