## Description

We are seeking a Physical Therapist (PT) to join our outpatient team on weekends. As a Physical Therapist at Advocate Health, you will provide high-quality patient care, evaluate patients, and develop treatment plans to help them achieve their goals.

**Job Summary:** Provide weekend support with flexibility to work Thursday-Saturday or Friday-Sunday at one of our locations. Schedule is flexible to PRN or part-time hours.

**Responsibilities:**

- Evaluate patients, select and administer assessment techniques, and synthesize information to establish patient care plans

- Gather and interpret data from medical records, physician reports, patients, and families

- Establish functional goals with patients and provide evidence-based interventions

- Document evaluations, progress notes, and discharge notes

- Communicate with physicians, referral sources, and clinical staff

- Supervise PTAs and Rehab Tech/Aides as required

**Physical Requirements:**

- Hearing and vision adequate for communication and reading

- Intelligible speech and adequate language/cognitive skills

- Ability to sit, stand, walk, climb stairs, kneel, twist, bend, and lift patients and equipment

- Personal Protective Equipment may be required

**Education, Experience, and Certifications:**

- Bachelor's degree in Physical Therapy

- Applicable state license for Physical Therapy

- BLS required
