## Description

First Community Health and Care is seeking a Rehabilitation Assistant to join the Intermediate Care team in Redhill. The successful candidate will work collaboratively with the multidisciplinary team, including Physiotherapists, Occupational Therapists, Nurses, and other Rehabilitation Assistants, to provide short-term care packages and rehabilitation to patients in their own homes.

The Rehabilitation Assistant will work under the direction of nurses and therapists, carrying out delegated tasks to promote independence and support patients in achieving their goals. The post involves providing care visits and rehabilitation in patients' own homes, requiring a car suitable for business use and a driving license.

Key responsibilities include:

- Working autonomously and as part of a team

- Managing time effectively

- Undertaking competencies to enhance skills in the role

- Having a car and driving license for work purposes

- An interest in rehabilitation to improve patient independence and achieve goals

The service operates 24/7, 365 days a year, and the post holder will be expected to work on a rota basis. Training and development opportunities are supported through an annual appraisal process and in-house training.

The salary for this role is £25,760 to £27,476 per annum, plus 5% HCAS per annum, pro-rata.
