## Description

We are seeking a Pediatric Physical Therapist to join our team at Advocate Condell Medical Center - Outpatient Rehabilitation. As a Pediatric Physical Therapist, you will work on a per diem basis, with a minimum of 8 hours per week, and provide high-quality patient care to pediatric patients.

**Responsibilities:**

- Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations

- Establish a plan of care and implement individual treatment based on evaluation results and functional goals

- Monitor and evaluate outcomes and adjust the individualized treatment plan as needed

- Provide written home programs, patient/family teaching, and referral for community services

- Communicate and collaborate with stakeholders to achieve optimum care for patients

- Complete all required documentation and maintain site/department established productivity standards

**Requirements:**

- Doctorate degree in Physical Therapy from an accredited school

- Licensed Physical Therapist in the State of Illinois

- Basic Life Support (BLS) for Healthcare Providers certification

- At least 1 year of experience as a Pediatric Physical Therapist

- Intermediate computer skills and excellent communication, interpersonal, and organization skills

**Physical Requirements and Working Conditions:**

- Ability to sit, stand, walk, lift, and squat throughout the workday

- Lift up to 50 lbs. from floor to waist and up to 10 lbs. from waist to overhead

- Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
