## Description

We are seeking a Full-time Home Health Physical Therapist to join our team in Round Lake and surrounding communities. As a Physical Therapist, you will play a crucial role in assuring and improving the quality of care for our patients.

**Responsibilities:**

- Assess patients and develop plans of care to achieve optimal health outcomes

- Provide high-quality, patient-centered care, exceeding expectations and promoting patient/family knowledge and confidence

- Collaborate with interdisciplinary teams to formulate integrated approaches to care

- Participate in quality/process improvement initiatives and promote efforts to reduce risk and improve patient safety

- Develop and maintain a restorative relationship with patients throughout the healthcare continuum

**Requirements:**

- Doctorate degree in Physical Therapy from an accredited school

- 1 year of clinical experience or strong spectrum of clinical affiliation experience

- Physical Therapy License in the State of Illinois or Wisconsin

- Basic Life Support (BLS) for Healthcare Providers

- Valid driver's license

- Ability to lift up to 35 pounds and work in various environments, including patient homes and assisted living facilities
