The Nurse Case Manager is responsible for coordinating case management activities with members to evaluate medical needs and facilitate overall wellness.

**Key Responsibilities:**

- Assess, plan, implement, and coordinate case management activities with members to evaluate medical needs and facilitate overall wellness.

- Develop a proactive course of action to address issues and enhance short-term and long-term outcomes.

- Conduct evaluations of members' needs and benefit plan eligibility.

- Apply clinical judgment to incorporate strategies designed to reduce risk factors and barriers.

- Assess the need for referrals to clinical resources.

- Consult with supervisors and others to overcome barriers in meeting goals and objectives.

- Utilize case management processes in compliance with regulatory and company policies and procedures.

**Requirements:**

- Minimum 3 years of clinical practice experience.

- Minimum 1 year of experience using MS Office Suite applications.

- Current active unrestricted RN licensure in the state of IL.

- Reliable transportation required; mileage is reimbursed.

- Ability to travel 50-75% of the time to meet members face-to-face in Northern suburbs of Chicago.

**Preferred Qualifications:**

- Case management experience in an integrated model.

- Experience working with adults with behavioral health needs.

**Education:**

- Associate's degree with RN licensure.

- Bachelor's of Science in Nursing preferred.

**Salary:** $66,575.00 - $142,576.00 per year
