## Description

We are seeking a Physical Therapist (PT) Float to join our innovative float team at Aurora in the Greater Milwaukee area. As a float therapist, you will have the opportunity to treat patients at outpatient clinic locations within a 45-mile radius of your home site.

**Responsibilities:**

- Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations

- Establish a plan of care and implement individual treatment based on evaluation results and functional goals

- Monitor and evaluate patient outcomes, adjusting the treatment plan as needed

- Plan for and complete patient discharge, including providing written home programs and patient/family teaching

- Complete all required documentation, including patient progress notes and discharge summaries

- Participate in performance improvement activities and pursue professional growth through continuing education

**Requirements:**

- Physical Therapist license issued by the state in which you practice (Wisconsin or Illinois)

- Basic Life Support (BLS) for Healthcare Providers certification

- Bachelor's Degree in Physical Therapy

- Intermediate computer skills and excellent communication and interpersonal skills

**Physical Requirements:**

- Ability to sit, stand, walk, lift, and squat throughout the workday

- Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height

- Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present
