## Description

Central London Community Health Trust is recruiting an experienced and motivated Band 6 Registered Nurse to join our established Watford and Three Rivers Community Nursing Team as a Deputy Team Lead.

The role offers an excellent opportunity to co-lead one of our neighbourhood teams, working closely with and supported by experienced Team Leaders.

The position is varied and rewarding, with responsibility for managing a patient caseload. You will assess, plan, implement and evaluate evidence-based care for patients in their own homes, helping to support independence and improve outcomes.

Key duties within the role include:

- Setting and maintaining standards of care and deploying, co-ordinating and supervising the nursing team and learners as required

- Providing leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys

- Being responsible for the team's assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community

- Maintaining patient and other records

- Participating in the long-term management of patients within the locality

- Having continued responsibility for a defined caseload

- Being responsible for the development and maintenance of competencies within the team

The salary for this role is £41,957 to £50,387 a year.

We are looking for someone who is committed to collaborative working, approachable, and an effective communicator. You will be confident working both independently and as part of a team, and open to new ideas and ways of working.

As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

The Trust offers a competitive employment package, including investment in its people, support for development, and help to thrive while delivering high-quality care.
