**Job Description:**

Join Oak Street Health’s NP Fellowship in Adult Primary Care, a 12-month paid fellowship that provides all the necessary skills, support, and learning required to become a highly confident and competent clinician for older adults.

During the program, fellows spend time building their own patient panel, learning from a multitude of preceptors, and engaging in didactic content that aims to build on, not repeat, graduate training.

**Responsibilities:**

- Build a patient panel and work with a multidisciplinary team to provide high-quality care

- Learn from experienced preceptors and engage in didactic content

- Develop skills and competencies required to become a confident and competent clinician

**Requirements:**

- Family, adult, or adult-gerontology primary care nurse practitioners or physician assistants who graduated or will graduate from their programs between May 2025 and December 2026 (NPs) or May 2025 and November 2026 (PAs)

- Minimum of one year experience, within the last five years, working as an adult RN (for NPs) or in healthcare (for PAs)

- Minimum GPA of 3.0 in NP/PA program

- Have passed, or in the process of, passing NP/PA board examinations by 12/7/26

- Currently holds or will hold by 12/21/26 an active, non-probationary state NP/PA license

- Bilingual Spanish required
