## Description

We are seeking a Physical Therapist to join our team in a small rural hospital setting.

The successful candidate will be responsible for interpreting physician referrals, completing musculoskeletal evaluations, establishing plans of care, monitoring patient outcomes, and adjusting treatment plans as needed.

Key responsibilities include:

- Interpreting physician referrals and completing evaluations

- Establishing plans of care and implementing individual treatment

- Monitoring and evaluating patient outcomes and adjusting treatment plans

- Planning for patient discharge, including providing written home programs and ordering equipment

- Completing required documentation and maintaining accurate records

- Cleaning up after patient treatment and keeping the area neat and orderly

- Demonstrating skill in the use of modalities, exercise equipment, and other devices

- Participating in performance improvement activities and managing assigned caseload

- Pursuing professional growth through continuing education and department inservices

Requirements:

- Physical Therapist license issued by the state of Wisconsin

- Basic Life Support (BLS) for Healthcare Providers certification

- Bachelor's Degree in Physical Therapy

- Intermediate computer skills, excellent communication and interpersonal skills, and demonstrated organization and problem-solving skills

Salary range: $41.10 - $61.65 per hour

Sign-on bonus: $20,000

Weekend premiums apply, with an additional $11 hourly for Fridays, Saturdays, and Sundays.
