## Description

We are seeking a skilled Physical Therapist to join our team at Advocate Health, specifically at the GCMG Children's Specialty Center Outpatient Therapy department.

As a Physical Therapist, you will play a crucial role in evaluating patients, selecting and administering assessment techniques, and developing patient care plans.

**Essential Functions:**

- Evaluate patients and create personalised care plans

- Gather and interpret data from medical records, physician reports, and patient interactions

- Establish functional goals with patients and provide evidence-based interventions

- Document patient evaluations, progress, and treatment plans

- Communicate with physicians, referral sources, and clinical staff

- Supervise PTAs and Rehab Tech/Aides as required

**Physical Requirements:**

- Hearing and vision adequate for communication and reading

- Intelligible speech and sufficient language/cognitive skills

- Ability to sit, stand, walk, climb stairs, kneel, twist, bend, and lift patients/equipment

- Occasional crouching, crawling, and reaching overhead

**Education, Experience, and Certifications:**

- Bachelor's degree in Physical Therapy

- Applicable state license for Physical Therapy

- BLS certification
