## Description

We are seeking enthusiastic and motivated Social Workers to join our Adult Learning Disabilities Team. As a Social Worker, you will manage a varied caseload and undertake assessments, reviews and support planning in line with the Care Act 2014.

The role includes:

- Completing Care Act assessments, reviews and support plans.

- Managing safeguarding enquiries and promoting positive risk management.

- Working within a strengths-based and person-centred framework.

- Undertaking Mental Capacity Act assessments and Best Interest processes where required.

- Building effective partnerships with health colleagues and community organisations.

- Supporting adults with learning disabilities to access services, opportunities and resources that improve wellbeing and quality of life.

- Maintaining accurate records and delivering high quality social work practice in line with professional standards.

**Salary:** £46,422 pa rising in annual increments to £52,920 pa inclusive of LW
