## Description

Coordinates bereavement program and related services for a facility or service line, including community grief support groups and outreach events.

Provides direct counseling and support to patients and families as part of the bereavement program services.

Evaluates bereavement activities and services, educates program staff and volunteers, and community members on addressing grief and loss.

Provides counseling and psychosocial assessment for grieving and soon-to-be-bereaved families, referring them to community agencies as needed.

Coordinates site or service line programs, such as maintaining the bereavement database, arranging mailings, and event planning.

May develop treatment plans for patient/family intervention, coordinate with team members, and prepare clinical and progress reports.

Collaborates with management to educate the community about the bereavement process and raises awareness of the communal obligation to those bereaved.

Develops relationships with community bereavement resources and stays current with bereavement literature and best practices.

Assesses and implements local bereavement quality improvement initiatives.
