## Description

We are seeking a part-time Home Care Physical Therapist to join our team in Cleveland. As a Home Care Physical Therapist, you will be responsible for managing a caseload of patients, ensuring timely care and documentation, and collaborating with a multidisciplinary team to promote health and safe outcomes.

**Major Responsibilities:**

- Manage daily patient caseload, ensuring timely care and documentation

- Collect and analyze data to determine patient needs and resources

- Prioritize multi-issue situations based on patient health and welfare

- Evaluate patients to determine rehabilitation needs and develop plans of care

- Establish and implement plans of care, performing physical therapy services and directing care coordination

- Maintain communication with multidisciplinary teams and provide updates on patient conditions

- Incorporate short and long-term goals into therapy and document progress

**Requirements:**

- Doctorate degree in Physical Therapy from an accredited school

- Physical Therapy License in the applicable state

- Basic Life Support for Healthcare Provider certification

- Current state driver's license and auto insurance

- Personal vehicle in safe driving condition

**Preferred Requirements:**

- 1-year experience as a licensed PT in a clinical care setting or home health

- Pediatric experience within the past 5 years
