## Description

We are seeking a Clinical Pharmacist to join our team at Intermountain Medical Center. As a Clinical Pharmacist in Abdominal Transplant, you will be responsible for providing pharmaceutical care to kidney and liver transplant recipients. Your duties will include inpatient management of abdominal transplant patients as well as ambulatory care services in the Transplant Clinic.

**Responsibilities:**

- Review patients' medication orders for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug-drug interactions

- Make evidence-based interventions to optimize medication use

- Provide counseling to patients and promote quality medication education and adherence

- Supervise and direct activities of interns, technicians, and support personnel as required

- Take the initiative to solve problems, including equipment, maintenance, drug availability, staffing issues, and extraordinary patient care concerns

- Identify potential areas for improvement, including processes that could be streamlined or revised to improve patient satisfaction

- Support clinic or hospital and system directives, including clinical, compliance, business operations, standardization, and customer service

**Requirements:**

- Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution

- License to practice pharmacy in the state of Utah and dispense controlled substances

- Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy in Utah

- Effective communication, people management, and project management skills

**Preferred Qualifications:**

- Experience in solid organ transplant

- Completion of a PGY2 Solid Organ Transplant Pharmacy residency or 3 years of transplant experience

- Previous experience working in an integrated healthcare delivery system

- Board Certification

- Health System Residency

**Schedule:**

- Monday through Friday from 7-3:30pm

- Weekends and holidays are required, with one weekend about every 7 weeks and one major or minor holiday each year

**Salary:** $62.89 - $92.44 per hour
