## Description

We are seeking a part-time Home Health Physical Therapist to join our team in East Grand Forks, MN. As a Physical Therapist, you will plan, organize, and deliver physical therapy programs to help patients recover and improve their mobility, relieve pain, increase strength, and prevent deformities.

Responsibilities:

- Conduct examinations, evaluations, and interventions for patients affected by injury or disease

- Develop and implement therapeutic plans, modifying them as necessary

- Educate patients and their family members about treatment plans and self-exercises to be continued at home

- Utilize tools and techniques to administer physical therapy interventions following safeguards

- Document patient information for evaluation and establish therapeutic plans

Requirements:

- ND & MN license required as a Physical Therapist

- Appropriate education level required in accordance with state licensure

- Basic Life Support (BLS) for healthcare providers required or within the first 60 days of employment

- Valid driver's license may be required
