## Description

We are seeking an enthusiastic, highly motivated and experienced Lead Practice Nurse to lead and develop our nursing team. As a Lead Practice Nurse, you will provide professional and clinical leadership to the nursing team, including Practice Nurses and Healthcare Assistants. Your key responsibilities will include:

- Providing professional and clinical leadership to the nursing team

- Supporting, mentoring and developing nursing colleagues

- Coordinating nursing services and supporting effective team working

- Promoting best practice, clinical excellence and a positive working environment

- Delivering high-quality nursing care and consultations

- Leading and supporting chronic disease management, including diabetes, asthma, COPD and hypertension

- Undertaking cervical screening, immunisations and vaccinations

- Providing wound management, health screening and preventative healthcare

- Supporting infection prevention and control

- Promoting patient education, health improvement and self-management

We offer a competitive salary of £48,000 to £53,000, dependent on experience and qualifications, as well as:
