## Description

Central and North West London NHS Foundation Trust is seeking a Mental Health Practitioner to join their Health & Justice team at HMP Downview. The successful candidate will be responsible for assessing and treating patients through their journey in prison, accepting referrals and triaging patients with the team, and undertaking comprehensive assessments and care planning.

The post holder will work within the Mental Health In-reach Team, providing effective case management to a defined caseload in accordance with the Care Programme Approach. They will also participate in Care and Separation Unit rounds with a GP and primary care nurse, and provide advice and information to patients about their care.
