## Description

The Care Manager, Telephonic Nurse 2 works in a telephonic environment, assessing and evaluating members' needs and requirements to achieve and maintain an optimal wellness state.

The role involves employing various strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.

Key responsibilities include:

- Identifying and resolving barriers to effective care

- Ensuring patients progress towards desired outcomes through continuous monitoring and active care planning

- Navigating multiple systems and using critical thinking skills

- Working with an assigned member population over the phone to perform health assessments, care planning, and education on chronic conditions and gaps in healthcare plans.

**Required Qualifications:**

- Bachelor's degree

- 3+ years of clinical acute care experience

- Licensed Registered Nurse (RN) in the state of Illinois with no disciplinary action

- Comprehensive knowledge of Microsoft Office applications, including Word, Excel, and Outlook

**Preferred Qualifications:**

- Experience with case management, discharge planning, and patient education for adult acute care

- Managed care experience

- Certified Case Manager (CCM)

- Bi-lingual (English/Spanish)

**Work Arrangements:**

- Remote position with possible occasional travel to Humana's offices for training or meetings

- Must have a dedicated workspace lacking ongoing interruptions to protect member PHI/HIPAA information

- Reliable internet service with a minimum download speed of 25 Mbps and an upload speed of 10 Mbps

**Scheduled Weekly Hours:** 40

**Pay Range:** $71,100 - $97,800 per year
