## Description

We are seeking a full-time Physical Therapist to join our team at Advocate Sherman Hospital - Acute Care Rehabilitation.

The Physical Therapist will interpret physician referrals, complete evaluations, establish plans of care, and implement individual treatments based on evaluation results and functional goals.

Responsibilities:

- Conduct musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations

- Develop and implement individualized treatment plans

- Monitor and evaluate patient outcomes, adjusting treatment plans as needed

- Provide patient/family teaching, referral for community services, and transition planning

- Complete required documentation, including patient evaluations, progress notes, and discharge summaries

- Participate in performance improvement activities and maintain productivity standards

Requirements:

- Doctorate degree in Physical Therapy from an accredited school

- Licensed Physical Therapist in the state of practice

- Basic Life Support (BLS) certification

- Intermediate computer skills and excellent communication, interpersonal, and organization skills

- Ability to lift, stand, walk, and squat throughout the workday

Preferred qualifications:

- Previous acute care experience

- Experience with EPIC EMR

Schedule details:

- Full-time position, 5 days/week (Monday - Friday)

- 2 evening shifts per week until 7:30 p.m. (Mondays and Thursdays)

- 1 weekend day shift every 4 weeks

- 1 summer and 1 winter holiday or weekend around a holiday

Compensation:

- $41.10 - $61.65 per hour

- $5000 sign-on bonus for full-time hires
