## Description

Central and North West London NHS Foundation Trust is seeking a Band 5 Community Staff Nurse for a 12-month fixed-term opportunity.

The successful candidate will join the Hillingdon District Nursing team, working autonomously in a community setting. You will need to be an NMC-registered nurse with a passion for delivering high-quality nursing care.

**Key Responsibilities:**

- Work as an autonomous practitioner in the community setting

- Provide diabetic management, including blood sugar monitoring and insulin administration

- Perform tissue viability and wound care, including leg ulcer management and compression therapy

- Develop bladder and bowel skills, including catheterization and bowel care

- Administer IV medication and provide palliative care

- Work closely with the multidisciplinary team and specialist services

**Requirements:**

- NMC registration as a Registered General Nurse (RGN)

- Evidence of working within multidisciplinary teams

- Demonstrable post-graduate experience in a relevant area working with people with long-term conditions

- Understanding of case management, case-finding systems, and care coordination

- Ability to maintain high standards of care and act as a role model

- Excellent communication and organizational skills
