## Description

We are seeking a skilled Physical Therapist (PT) to join our outpatient team at Advocate Health in Marinette, WI. As a Physical Therapist, you will play a crucial role in providing high-quality patient care, evaluating patients, developing treatment plans, and monitoring progress.

**Responsibilities:**

- Interpret physician referrals and complete evaluations using appropriate methods

- Establish a plan of care and implement individual treatment based on evaluation results and functional goals

- Monitor and evaluate outcomes, adjusting the treatment plan as needed

- Provide written home programs, patient/family teaching, and referral for community services

- Communicate and collaborate with stakeholders to achieve optimum care for patients

- Complete all required documentation and maintain site/department established productivity standards

- Participate in performance improvement activities and pursue professional growth through continuing education courses

**Requirements:**

- Doctorate degree in Physical Therapy from an accredited school

- Licensed Physical Therapist in the state of practice

- Basic Life Support (BLS) for Healthcare Providers certification

- Intermediate computer skills and excellent communication, interpersonal, and organization skills

- Ability to lift up to 50 lbs., stand, walk, and sit throughout the workday

**Preferred Qualifications:**

- Pediatric Physical Therapist preferred

- Experience working in a rural outpatient clinic
