The Physical Therapist will treat adult patients throughout the hospital, excluding the inpatient rehab floor and pediatrics. The team consists of PT, OT, SLP, and rehab aides. **Major Responsibilities:** - Interpret physician referrals and complete evaluations using appropriate methods. - Establish a plan of care and implement individual treatment based on evaluation results and functional goals. - Monitor and evaluate outcomes, adjust the treatment plan as needed, and provide status updates to physicians and other teammates. - Identify factors affecting patient's physical and psychosocial comfort and attend to these needs. - Plan for and complete patient discharge, including providing written home programs, patient/family teaching, and referral for community services. - Communicate and collaborate with stakeholders to achieve optimum care for the patient. - Complete all required documentation, including patient evaluations, progress notes, and discharge summaries. - Maintain site/department established productivity standards and adhere to billing and documentation policies. - Clean up after patient treatment, put away equipment, and keep the area neat and orderly. - Perform various manual techniques, patient positioning, transferring, and transporting duties. - Demonstrate skill in use of modalities, exercise equipment, and other devices. - Promote patient safety by participating in safety initiatives and following all safety and infection control measures. - Participate in performance improvement activities and manage assigned caseload. - Pursue professional growth through continuing education courses, department in-services, and reading professional literature. - Orient new staff to clinical and department policies as requested. - Participate in the evaluation process of co-workers. - May supervise physical therapy assistants, physical therapy extenders, and physical therapy students. **Minimum Job Requirements:** - Doctorate degree in Physical Therapy from an accredited school. - Licensed Physical Therapist in the state where you work. - Basic Life Support (BLS) for Healthcare Providers certification. - Intermediate computer skills. - Excellent communication, interpersonal, and organization skills. - Demonstrated organization, decision-making, and problem-solving skills. **Physical Requirements and Working Conditions:** - Must be able to sit, stand, walk, lift, and squat throughout the workday. - Lift up to 50 lbs. from floor to waist, up to 10 lbs. from waist to overhead, and carry up to 40 lbs. at waist height. - Push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present. - Repetitive use of hands for simple grasping, pushing/pulling, and fine manipulation. - Functional use of senses to allow for effective communication. - Exposed to electrical and chemical hazards, blood and body fluids, and road and weather hazards. **What We Offer:** - Competitive compensation and benefits package. - Paid Time Off programs. - Health and welfare benefits. - Flexible Spending Accounts. - Family benefits. - Defined contribution retirement plans with employer match. - Educational Assistance Program. ## Skills ### Required - Physical Therapy - Patient Evaluation - Treatment Planning - Communication - Documentation - Billing - Safety Protocols --- Source: [Apply at aah.wd5.myworkdayjobs.com](https://aah.wd5.myworkdayjobs.com/en-US/External/job/Advocate-Lutheran-General-Hospital---1775-Dempster-St/Physical-Therapy-Rehab-Registry_R216279?utm_source=yubhub.co&utm_medium=jobs_feed&utm_campaign=apply)
