## Description

We are seeking a full-time Home Care Physical Therapist to join our team in University City, North Carolina.

The successful candidate will be responsible for managing a caseload of patients, ensuring timely care and documentation, and collaborating with multidisciplinary teams to provide high-quality patient outcomes.

Responsibilities:

- Manage daily patient caseload, ensuring timely care and documentation

- Collect and analyze patient data to identify high-risk patients and develop effective care plans

- Prioritize patient needs and coordinate care with multidisciplinary teams

- Evaluate patients to determine rehabilitation needs and develop plans of care

- Implement plans of care, perform physical therapy services, and direct care coordination

- Maintain communication with teams, physicians, and patients

- Incorporate short- and long-term goals into therapy and document progress

Requirements:

- Doctor of Physical Therapy (DPT) degree required

- Physical Therapy License in the state of practice required

- Basic Life Support for Healthcare Provider certification required

- Current state driver's license and auto insurance required

- Personal vehicle must be well-maintained and in safe driving condition

Preferred Requirements:

- 1-year experience as a licensed PT in a clinical care setting or home health preferred

- Pediatric experience within the past 5 years preferred
