## Description

We are seeking a Physical Therapist to join our new at-home program serving outpatient therapy across the Northern Charlotte area.

As a Physical Therapist with Advocate Health, you will deliver high-quality, one-on-one outpatient physical therapy services in patients' homes, helping improve mobility, function, and independence.

**Responsibilities:**

- Evaluate patients and develop individualized treatment plans

- Provide evidence-based outpatient care, collaborating with physicians, caregivers, and interdisciplinary team members

- Utilize technology and flexible scheduling to manage a caseload across a defined geographic area

- Document evaluations, progress notes, and discharge notes as needed

- Communicate patient progress to physician, referral sources, and other clinical staff

**Requirements:**

- Bachelor's degree in Physical Therapy program required

- Applicable state license for Physical Therapy required

- BLS required per policy guidelines

- Ability to lift patients, equipment, or supplies up to 20 pounds frequently and 50 pounds occasionally
