## Description

We are seeking a full-time Physical Therapist to join our Inpatient Rehabilitation team at Wake Forest Baptist Medical Center. The successful candidate will be responsible for providing high-quality patient care, evaluating patients, developing treatment plans, and collaborating with interdisciplinary teams.

**Responsibilities:**

- Provide age-appropriate patient care, demonstrating knowledge of growth and development principles across the lifespan

- Evaluate patients, develop treatment plans, and implement evidence-based interventions

- Establish functional goals with patients and caregivers, monitor outcomes, and adjust treatment plans as needed

- Educate patients and caregivers on diagnosis, treatment, and home instructions

- Communicate effectively with healthcare professionals, patients, and caregivers

- Complete accurate and timely documentation, including patient evaluations, progress notes, and discharge summaries

- Participate in performance improvement initiatives and maintain a clean and organized work environment

**Requirements:**

- Physical Therapy license in the state of employment

- Basic Life Support (BLS) certification for Healthcare Providers

- Degree in Physical Therapy from an accredited institution

- Continuing education requirements as mandated by applicable state professional standards
