## Description

We are seeking a Physical Therapist, Float, Outpatient to join our team at Advocate Health. As a Physical Therapist, you will provide high-quality patient care, evaluate patients, and develop treatment plans to enhance physical and functional abilities.

**Responsibilities:**

- Provide age-appropriate patient care, demonstrating knowledge of growth and development principles across the lifespan

- Evaluate patients, interpret medical records, and establish patient care plans using evidence-based interventions

- Set functional goals with patients and caregivers, monitor outcomes, and adjust treatment plans as needed

- Educate patients and caregivers on diagnosis, treatment, and home instructions

- Communicate effectively with healthcare providers, staff, and external customers

- Complete required documentation, maintain productivity standards, and adhere to billing and documentation policies

- Participate in performance improvement initiatives, professional development, and continuing education

- Meet requirements for the Enterprise Float Program, demonstrating competence across assigned departments and locations

**Requirements:**

- Degree in Physical Therapy from an accredited institution

- Doctor of Physical Therapy (DPT) preferred or commensurate educational requirements

- Physical Therapy License in the state(s) of employment

- Basic Life Support (BLS) certification for Healthcare Providers

- Intermediate computer skills and excellent communication, interpersonal, and organizational skills
