## Description

We are seeking a Physical Therapist to join our Home Health team in the Far Southwest Suburbs, focusing on Midlothian, Crestwood, Robbins, Blue Island, Calumet Park, and Alsip. As a Physical Therapist, you will provide high-quality patient care, facilitate quality improvement initiatives, and collaborate with the healthcare team to achieve optimal health outcomes.

**Responsibilities:**

- Assess patients' physical needs and develop plans of care

- Provide skilled physical therapy interventions, including exercises and education

- Collaborate with patients, families, and healthcare teams to achieve goals

- Participate in quality improvement initiatives and adhere to organisational policies

- Maintain accurate records and documentation

**Requirements:**

- Licensed Physical Therapist in the State of Illinois (or eligible for licensure)

- 1 year of clinical experience or strong clinical affiliation experience

- Basic computer skills, including Microsoft Word and email

- Ability to communicate professionally and work effectively with patients and healthcare teams

- Strong time management and organisational skills

**Schedule:**

- Monday - Friday, 8am - 5pm

- Rotating weekends (every 5 weeks) and holidays (1 summer and 1 winter holiday per year)
