## Description

The Palliative Care Social Work Care Manager I provides psychosocial assessments, develops care plans, and offers clinical counseling. Responsibilities include assessing patients for transition planning, mental health, and substance use; developing and monitoring care plans; providing brief therapy; identifying mental health disorders; and connecting patients to services.

The role involves working with patients, support persons, healthcare providers, insurers, and community resources. Key tasks are:

- Evaluating patients for various needs

- Developing and monitoring care plans

- Providing therapeutic interventions

- Identifying mental health disorders and making referrals

- Educating patients and staff

- Collaborating with healthcare teams and insurers

- Leading improvement initiatives and tracking metrics

- Facilitating advanced care planning discussions

Requirements include a Master's in Social Work, current state licensure, basic computer skills, strong communication skills, and critical thinking ability. Preferred qualifications include case management certification, experience in clinical care management, and understanding of health insurance products.

The position offers a salary range of $39.16 - $60.42 per hour and is full-time, Monday to Friday, 8:30am - 5:00pm. Benefits include a comprehensive package covering wellness programs.
