## Description

We are seeking a Physical Therapist to join our team at Advocate Illinois Masonic Medical Center - Acute Care Rehabilitation.

As a Physical Therapist, you will interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations. You will establish a plan of care, implement individual treatment, monitor and evaluate outcomes, and adjust the treatment plan as needed.

Responsibilities:

- Interpret physician referrals and complete evaluations

- Establish a plan of care and implement individual treatment

- Monitor and evaluate outcomes and adjust the treatment plan

- Identify factors affecting patient's physical and psychosocial comfort

- Plan for and complete patient discharge

- Communicate and collaborate with stakeholders

- Complete required documentation and maintain productivity standards

- Participate in performance improvement activities

- Pursue professional growth through continuing education

Requirements:

- Doctorate degree in Physical Therapy from an accredited school

- Licensed Physical Therapist in the State

- Basic Life Support (BLS) for Healthcare Providers

- Intermediate computer skills

- Excellent communication, interpersonal, and organization skills

Physical Requirements:

- Ability to sit, stand, walk, lift, and squat throughout the workday

- Lift up to 50 lbs. from floor to waist

- Lift up to 10 lbs. from waist to overhead

- Push/pull with 30 lbs. of force
