## Description

We are seeking a Physical Therapist to join our team at Aspirus Divine Savior Wellness Center in Portage, WI. As a Physical Therapist, you will provide physical therapy to patients of all ages, evaluate and establish treatment plans, and provide documentation for all treatments.

**Responsibilities:**

- Evaluate patients and establish treatment plans

- Provide physical therapy to patients of all ages

- Develop and implement treatment modalities and procedures

- Provide patient and family education

- Maintain accurate documentation

**Experience and Qualifications:**

- Minimum of a bachelor's degree in physical therapy from a school accredited by the American Physical Therapy Association (APTA)

- Current Wisconsin and/or Michigan licensure to practice Physical Therapy

- Current BLS certification or obtained within 90 days of hire

- Experience in general practice of physical therapy with experience in area of specialization beneficial
