## Description

We are seeking a Physical Therapist (PT) Float to join our innovative team at Aurora! As a float therapist, you will have the opportunity to treat patients at outpatient clinic locations within a 45-minute drive from your home.

**Responsibilities:**

- Interpret physician referrals and complete evaluations

- Establish a plan of care and implement individual treatment

- Monitor and evaluate outcomes, adjusting the treatment plan as needed

- Plan for and complete patient discharge, including providing written home programs and patient/family teaching

- Complete all required documentation, including patient progress notes and discharge summaries

- Participate in performance improvement activities and pursue professional growth through continuing education

**Requirements:**

- Physical Therapist license issued by the state in which you practice (Wisconsin or Illinois)

- Basic Life Support (BLS) for Healthcare Providers certification

- Bachelor's Degree in Physical Therapy

- Intermediate computer skills and excellent communication and interpersonal skills

**Benefits:**

- Competitive salary range: $41.10 - $61.65 per hour

- Sign-on bonus: $10,000 for full-time new hires or $5,000 for part-time new hires

- $5/hour float differential
