## Description

The Clinical Social Worker utilises clinical expertise to perform psychosocial assessments, develop and implement plans of care, assess crisis situations, and provide family and individual treatment.

This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties.

**Sign-on Bonus:** $2500 (external candidates only)

**Responsibilities:**

- Assess the patient's medical history, mental health history, and current presentation to understand the patient's needs from treatment.

- Diagnose and treat various mental, emotional, and behavioural disorders.

- Develop a plan of care either individually or with input from the patient, patient family/significant others, physicians, and other members of the healthcare team.

- Provide treatment based on patient need and evidence-based interventions.

- Refer patients to other services to improve their well-being.

- Properly document pertinent information regarding the patient's plan of care.

- Consult with the multi-disciplinary team to develop treatment plans, behaviour plans, and discharge plans.

**Requirements:**

- Master's degree in social work or related field from an accredited institution.

- Current state licensure as LCSW, LMFT, or CMHC.

- Excellent written and verbal communication skills.

- Computer skills, i.e., email, word processing.

**Preferred Qualifications:**

- PSYPACT eligible or PSYPACT authorization
