## Description

We are seeking an experienced and dynamic clinician to join the Lambeth Continuing Healthcare Team as Clinical Lead (Band 8a). This is an exciting opportunity to play a pivotal leadership role within a service that sits at the heart of integrated health and care delivery.

The successful candidate will help shape and deliver high-quality NHS Continuing Healthcare services for Lambeth residents, working across organisational boundaries to improve outcomes, strengthen partnerships and ensure effective use of NHS resources.

Main duties of the job:

- Provide expert clinical leadership, advice and assurance to SEL ICB (Lambeth Place) and the CHC service.

- Lead on the delivery of safe, effective and person-centred NHS Continuing Healthcare services in line with the National Framework.

- Support decision-making processes relating to CHC eligibility, reviews, appeals and complex case management.

- Undertake clinical review and scrutiny of CHC-funded care packages, ensuring quality, value for money and person-centred outcomes.

- Provide clinical oversight of complex cases, including individuals with highly complex health, behavioural or safeguarding needs.

- Work collaboratively with community services, acute providers, primary care, mental health services, social care and independent sector providers to deliver joined-up care.

- Lead and support multi-disciplinary discussions regarding eligibility, care planning and commissioning arrangements.

- Contribute to service improvement, quality assurance and performance management, including supporting continued improvements in 28-day performance and other key national metrics.

- Provide specialist advice on complex commissioning decisions, individual funding arrangements and care package optimisation.

- Support the development, supervision and professional leadership of CHC nurses and wider team members.

- Lead on clinical governance, safeguarding, risk management and quality monitoring activities.

The Lambeth Place-based Team works in partnership with the London Borough of Lambeth and local providers to commission and oversee services that meet the needs of local people.

The salary for this role is £66,274 to £73,496 a year per annum inclusive of Inner London HCAS.

## Skills

### Required
- Registered Nurse
- Master's degree in health, nursing, social care
- Significant NHS Continuing Healthcare (CHC) experience
- Specialist knowledge of complex care, safeguarding, MCA, DoLS and clinical risk management

### Nice to have
- Leadership/management and/or teaching qualification
