## Description

The Transitions and Leaving Care Service works with young people aged 16-25 who are in care and those who were previously in care. As a Personal Advisor, you will provide information, advice, guidance, and support to enable young people to make a successful transition into adulthood.

Key responsibilities include:

- Providing information, advice, and guidance based on individual needs

- Completing assessments of need to inform the Pathway Plan

- Taking a lead role in preparing and completing the Pathway Plan

- Regularly monitoring progress and facilitating reviews

- Liaising with various agencies and advocating for the young person

- Coordinating service provision and encouraging access to services

- Building and maintaining supportive relationships with care leavers

- Maintaining regular contact with young people, potentially travelling throughout the UK and overseas

- Completing records of contact using the Signs of Safety model

- Providing support cover as part of the office duty rota

The successful candidate will be required to be office-based at least 3 days a week.
