## Description

Join our caring community as a Manager, Care Coaching. You will provide daily leadership and mentoring to Care Management staff, ensuring the delivery of high-quality services that express Humana values and Family Care program values.

**Responsibilities:**

- Mentor and support Field Nurses and Care Coaches to ensure compliance with the DHS contract

- Identify learning needs and provide clear expectations, orientation, and ongoing learning to build capacity of IDT staff

- Assess and coach staff on IDT primary competencies and performance standards

- Review reports, assess processes, and review documentation

- Assess utilization and quality of care management for each member served

- Participate in cross-departmental collaboration to ensure quality and effectiveness of provider services

- Educate community partners

- Participate in hiring, orienting, and training associates

- Facilitate in-person unit meetings and attend quality visits

**Requirements:**

- Bachelor's degree in human services or a related field, or associate degree in nursing

- 1 year of leadership experience in human services

- 3 years of experience working in the Family Care program

**Preferred Qualifications:**

- 5 years of experience working with individuals with intellectual/developmental disabilities, physical disabilities, or frail elders

- Reside within 1 hour of Eau Claire

- Master's degree in public administration, social work, or a related field

**Work Details:**

- Work remotely from home

- Work location: Eau Claire

- Typical work days/hours: Monday – Friday, 8:00 am – 4:30 pm CST

- Travel: at least once/month, with potential for more travel

- Direct reports: approximately 11-15

**Compensation:**

- Salary range: $78,400 - $107,800 per year

- Eligible for a bonus incentive plan
