## Description

We are seeking 3 Social Workers to join our Adult Social Care team in London Borough of Hounslow.

The main role is to provide care and support to residents and carers, undertaking social care assessments, and identifying needs and desired outcomes.

Responsibilities:

- Provide care and support to residents and carers

- Undertake social care assessments

- Identify needs and desired outcomes

- Work as part of a team to provide high standard care

- Follow practices that comply with policies and procedures

Requirements:

- Knowledge of social care legislation, safeguarding, Mental Capacity Act, Care Act 2014

- Experience within statutory mental health services

- Commitment to person-centred, strengths-based approaches

- Understanding of Social Work England standards, equality, diversity, and safeguarding responsibilities
