## Description

Join CLCH Hertfordshire Adult Community Services as an Occupational Therapist on the Inpatient Rehabilitation Wards at Potters Bar Community Hospital.

As part of a multi-disciplinary team, you will support patients with a variety of conditions through comprehensive occupational therapy assessment, treatment and discharge planning.

In this Band 5 role, you will:

- Work collaboratively with colleagues to deliver patient-centred rehabilitation.

- Assess individual needs and plan appropriate occupational therapy interventions.

- Support safe, effective discharge planning.

Main duties of the job:

- Provide occupational therapy assessment and treatment for referred patients, planning, delivering and evaluating interventions for individuals and groups.

- Manage your own caseload autonomously in line with professional standards and clinical guidelines, maintaining accurate professional documentation.

- Contribute actively to the multi-disciplinary team, delivering a comprehensive Occupational Therapy service for residents of West Hertfordshire.

- Build and maintain effective relationships with referring CLCH acute and community services to support appropriate referrals and understanding of the Occupational Therapist and multi-professional team roles.

- Take part in Continuing Professional Development, in-service training, audit and research undertaken by the service.

## Skills

### Required
- Diploma/Degree in Occupational Therapy
- Current AHP with UK HCPC registration
- Evidence of active CPD portfolio
- Relevant Post graduate training
- Application of the OT Process
- Experience of working as part of an MDT
- Use of functional assessment and activity analysis

### Nice to have
- Experience of delegation to support staff
- Previous experience of working in the NHS
